STEP ONE

A caregiver—or a provider on behalf of a caregiver—reaches out to Help Me Grow Long Island to seek support.

A Family Resource Specialist completes an intake to understand and identify the family’s unique needs.

STEP THREE

The specialist identifies community resources and helps the family navigate next steps.
